Your first visit
You will spend about an hour with us during your first visit as we take a careful medical history, physical examination, and possibly perform skin testing. If you have breathing problems, we may also perform a lung function test. We also will begin the process of treating your condition with the proper medications, advising you on avoidance of possible allergens, and discussing whether allergy injections are right for you.
Before your first visit
Please remember to bring your insurance card, a picture I.D., and any necessary referrals. If you have any questions about whether or not you need referrals, please ask our office staff or your insurance provider at least two days prior to coming to the office.
Bring a list of your current medications and copies of any blood tests or X-rays that may be related to the reason you are seeing us. Also, you may wish to print out our new patient forms prior to coming.
Since we may be skin testing you, try to avoid taking antihistamines (allergy pills) for 3 to 5 days before the visit. If you have children coming in, you may wish to provide us with any medical forms needed for school or camp.

For prescription renewal requests, please allow 2 to 3 business days for the medications to be called in and include your name, date of birth, and pharmacy’s phone number. Also include the name of the medication, the dose, and directions. Click here for our Contacts & Locations
